There are currently over 200 full service restaurants in 29 states,
the District of Columbia and six foreign countries.
History/Company Profile:
On March 27, 1985, attorneys Rick Rosenfield and Larry Flax traded
in the courtroom for the dining room. Having always wanted to get
into the restaurant business, they pitched their legal pads to serve
hearth-baked pizzas. They introduced flavors and tastes from around
the world, from Thai to Jamaican Jerk all on a pizza. All of our innovative
pizzas are creatively designed on a delicious crust, and hearth-baked
to perfection. Now, California Pizza Kitchen (CPK) is a leading casual
dining chain in the premium pizza segment with a recognized consumer
brand and an established, loyal customer base. CPK's strong brand
awareness has been developed primarily through full service restaurants,
and through the expansion of our high quality fast-casual concept
called the CPK/ASAP. These CPK/ASAP restaurants are significantly
smaller than our full service restaurants and offer a limited selection
of only the most popular pizzas, salads, sandwiches and appetizers.
California Pizza Kitchen also has a strategic alliance with Kraft
Pizza Company, a subsidiary of Kraft Foods Inc., to distribute a line
of premium frozen pizzas through supermarkets and other retail outlets.
Menu:
Our menu features hearth-baked pizzas, innovative salads, creative
pastas, delicious deserts and much more.
Benefits:
Vacation:
Two weeks paid vacation after one year of employment, three weeks
after 5 years, and four weeks after 10 years.
Medical:
CPK offers Salaried employees the Blue Cross-Blue Card PPO Plan.
• PPO: With this option you select the provider of your choice.
If you choose in-network providers, the plan generally pays 90% of
covered expenses after you meet your deductible of $250.00. For a
Physician Visit using the PPO Provider Network your office visit is
$15.00. If you choose out-of-network providers the plan generally
pays 70% of covered expenses after you meet your deductible.
• Prescription Drugs – Prescription Drugs are covered
at $10.00 for a generic and $20.00 for brand name prescriptions. A
60-day supply may be ordered through the mail order benefit at a reduced
cost.
Dental:
Employees have a choice of two dental plans – Aetna DMO Dental
or MetLife PPO Plan.
DMO (Dental Maintenance Organization) Dental:
• You must select a Primary Care Dentist from the network and
see that dentist whenever you seek dental care.
• This plan has no deductible. Instead you pay a set co-payment
for each service that is provided. This plan tends to cost you less
out of pocket.
• All preventative care (x-rays and cleaning) is covered at
100%.
MetLife PPO Dental:
• If elected you may see any dentist of your choice on this
plan, however you receive added savings, if you see a MetLife PPO
Dentist.
• You must meet a $50 individual deductible ($150.00 family)
before the plan will pay 80% of general dental expense and 50% of
more involved “major” procedures for an annual maximum
of $2000 per person.
• All preventative care (x-rays and cleaning) is covered at
100%, deductible waived.
Vision:
CPK provides a comprehensive vision program through Spectera to our
salaried employees participating in the major medical and dental programs.
Highlights include: exams once every 12 months ($10 co-pay); new lenses
once every 12 months ($25 co-pay); and new frames once every 24 months
($25 co-pay, up to $110-$150 allowance). This plan includes allowances
for both in-and out-of-network providers, and a discount program for
laser eye surgery.

Optional Short/Long Term Disability Insurance:
The option is available for purchasing additional short-term disability
coverage. This coverage provides you with a benefit of 60% of your
weekly base salary up to a maximum of $750.00 per week and coordinates
with any state mandated plans in California, Hawaii, New Jersey and
New York. Short-term disability provides income replacement in the
event that you become ill or injured and are unable to work for more
than 7 days.
Salaried employees are automatically enrolled in the Long Term Disability
(LTD) Plan. In the event of your disability, LTD pays 60% of your
monthly wages up to $10,000 per month. Benefits begin after 90 days
of total disability. CPK pays the entire premium for this valuable
coverage.
Supplemental Life Insurance:
Life insurance and Accidental Dismemberment coverage is provided at
no additional cost to the employee when the employee enrolls in the
medical/dental coverage. Both coverage’s are provided in the
amount of 1x’s your annual salary (including bonus if applicable)
up to a maximum of $150,000.
401(k) Plan:
You may enroll in CPK’s Retirement Savings Plan, provided through
the Lincoln Alliance Program, after completing 6 months of service
as long as you are 21 years of age.
