Medical Benefits:

• Inpatient Hospital Services- $250 per admission
• Outpatient surgery- no charge
• Emergency room care- $100 co payment per visit, but is waived
if admitted.

Dental Benefits: Blue Cross of California
• Option of receiving treatment from any licensed dental provider
you choose
Vision Benefits: EyeMed
• If you go to a provider who belongs to EyeMed’s PPO
network you will receive a higher level of benefits and enjoy a greater
level of convenience. If you got o an out-of-network provider you
will be reimbursed for exams and eyewear according to the schedule
of allowances.
Life Insurance:
Bj’s will pay full cost of basic life insurance and AD&D
insurance though Blue Cross. Your benefit amount under each plan is
$25,000.
Payment Schedule: Monthly for benefits 3,4,5,&6
Employee: $60.00
Employee and Spouse: $241.00
Employee and Child(ren): $192.00
Employee and Family: $393.00
Long Term Disability:
60% of base earnings to a maximum monthly benefit of $6000.Your benefit
under the plan will be reduced by any other disability income benefits
you receive, such as Social Security, SDI, or Workers Compensation
benefits.
Supplemental Life:
$10,000 increments, up to a maximum benefit of five times your annual
earnings or $500,000 whichever is less. Premium is based on your age.
If you choose to purchase more then $150,000 worth of coverage, you
will be required to give Mutual of Omaha satisfactory evidence of
good health before coverage over that amount will become effective.
In addition you may purchase voluntary life insurance coverage in
$5000 increments, up to a maximum benefit of 50% of your voluntary
life insurance amount or $100,000 whichever is less. Coverage is also
available for dependent children in $1000 increments up to a maximum
benefit of $10,000.
401(k):
$0.33 to the dollar up to the first 6% of pay
